Self-exclusion is a voluntary program in the State of Arizona that allows you to request to be banned from entering and gambling at all tribal (Indian) casinos in Arizona.
It applies to all Indian Gaming Facilities in Arizona – meaning every tribal casino located in the state.
There is a Self-Exclusion Administrator – a designated person/office that:
answers your questions,
explains the process,
helps you complete your request.
📞 Phone: 602-290-2555
📧 Email: contact@problemgambling.az.gov
You are the one requesting the ban – it is not a punishment, but your personal decision for protection.
Once you are placed on the list, casinos must deny you access (you cannot gamble or stay inside those facilities).
The program is designed to help people who want to stop or control their gambling.
If gambling feels out of control.
If you want a “physical barrier” to prevent yourself from entering casinos where you might be tempted to gamble.
Call the administrator at 602-290-2555 or send an email to contact@problemgambling.az.gov.
Say: “I would like information about the Self-Exclusion program in Arizona and how to enroll.”
Follow the instructions – they will explain what you need to sign, which documents to bring (ID, driver’s license, photo), where to submit the request, and when the ban becomes effective.
Duration: How long does the self-exclusion last (1 year, 5 years, lifetime)?
Reversal: Can the decision be changed or ended early?
Coverage: Does it apply only in Arizona, or also in other states?
Online gambling: Is it limited to physical casinos, or does it include online operators?
Privacy: Who can see your information and how is it protected?
Violations: What happens if you try to enter a casino after signing up?
